I have read, and I understand, the Dixie State College of Utah Val A. Browning Library Course Reserve Policy & Procedures (http://library.dixie.edu/faculty/reserve.html).

My signature certifies that the items I am placing on Course Reserve meet at least one of the following criteria:

1. Falls under the Fair Use Provisions set forth in Section 17 of the United States Copyright Act of 1976.

2. I am the copyright owner, and I give the library permission to post these items on electronic course reserve.

3. I have documented permission from the copyright owner to use these materials.

4. The work is in the public domain.

By signing I agree that I am solely and wholly responsible for ensuring compliance with copyright law and that, as the instructor of the course listed below, I assume all liability for any and all violations.

I also agree that the library retains the right to refuse placement of materials on Course Reserve or Electronic Course Reserve if library staff determine the items are in violation of the Library Course Reserve Policy or Copyright Law. I understand that I am required to complete a new form for each semester an item is placed on Course Reserve or Electronic Course Reserve.
